2024 Terms of Registration

I am aware that the Burlington Old Timers Soccer Club Inc. (hereafter BOTSC) is not associated or otherwise affiliated with the Ontario Soccer Association or any other District Association.  I further acknowledge that I have been advised by the BOTSC that there is no insurance coverage being provided by the BOTSC regarding any participation which I may have in any events whether they be games or other events for injuries which I may sustain and that I am solely responsible for providing any such insurance coverage which I may wish to obtain for my own protection.

In consideration of the acceptance of my membership in the BOTSC, I hereby, for myself, my heirs, executors, administrators and assigns, waive any claims to which I may become entitled for injury or damage and hereby release the BOTSC and all other organizers, sponsors, representatives, members, game officials, their agents and employees and any other person or organization assisting from any claims, damages, actions or causes of action of action which I may have as a results of my participation in or traveling to or from programs, games or activities of the BOTSC.  I further state that I am in proper physical condition to participate in the events scheduled by the BOTSC and that I am aware that participation could, in some circumstances, result in physical injury.

I further acknowledge that I have been advised that I can seek my own independant legal advice with regard to the execution of this Release and I have chosen not to do so.
